American Pride wellness policy, goes into effect 1/1/14 

All changes and additions go into effect 1/1/2014.

--Will pay up to $30/month for a gym or YMCA membership through automatic deduction. Starts with 2nd month (proof of 30 days membership required).

--Will give all associates that participate in health and wellness program a $25 bonus on the last paycheck of each month. Program requires monthly coaching calls (about 15 minutes each) and a quarterly activity.

--All vaccines and annual preventative visits to your physician will be paid 100%, with no copay.

--All visits to "quick med" type clinics will be paid 100%, with no copay.

--All preferred generic prescriptions will be paid 100%, $0 copay.

--All nonpreferred generic prescriptions will be covered with a $0 copay.

--All preferred brand prescriptions will be covered with a $10 copay.

--All other brand name prescriptions will be covered with a $50 copay, with required quarterly review of possible alternatives through coaching/wellness service.

-- $25 copay for doctor/specialist/chiropractor visit.

--Health insurance remains 100% paid for FTassociate, PT pays $50 a biweekly paycheck

--For +1 coverage, FT pays $65 a biweekly paycheck, PT pays $160.

--For family coverage, FT pays $130 a biweekly paycheck, PT pays $200.
